Where do I find the employer authorization form?
In Client Resources, you can either print and fill out an authorization form by hand or fill it out and submit it digitally.

How is Total Occupational Medicine affiliated with Lake Urgent Care/Lake After Hours?
As part of our urgent care hybrid model, Total Occupational Medicine is affiliated with Lake Urgent Care/Lake After Hours to provide convenient occupational health services. Each urgent care offers area employers an array of near-site occupational health options. Services range from the treatment of workplace injuries and DOT/Non-DOT physicals to a wide variety of testing options and immunizations.

No, we are a walk-in facility. Total Occupational Medicine is here, providing professional treatment without appointments or lengthy waiting.

Is Total Occupational Medicine open on weekends?
Yes. Our partnership with Lake Urgent Care/Lake After Hours allows our patients to have access to occupational health needs on weekends. Total Occupational Medicine shares a building with the Lake After Hours Drusilla location, which is open 8:00am-11:00pm daily.

What is a Virtual Visit?
Virtual Visit allows our providers to evaluate, diagnose and treat patients at a distance using telecommunications technology. Total Occupational Medicine uses innovative, professional medical-grade devices with advanced HD video. Our providers can provide care remotely that is on par with an in-person visit, including conducting basic exams, checking vital and performing urgent care testing.
